As a firefighter with Shaker Road, Loudonville Fire Department, you will be expected to perform many tasks as part of the job. These tasks may include:
• climbing ladders
• lifting and carrying objects weighing over 100 pounds
• crawling on hands and knees
• using self-contained breathing apparatus
• working in extremely high temperatures, hazardous atmospheres and other dangerous situations
YOU WILL BE REQUIRED TO PASS A PHYSICAL EXAMINATION GIVEN BY SRLFD’S HEALTH CARE PROVIDER PRIOR TO BEING CONSIDERED FOR MEMBERSHIP. ALL COSTS WILL BE PAID FOR AND ARRANGEMENTS WILL BE MADE BY SHAKER ROAD LOUDONVILLE FIRE DEPARTMENT.
The Occupational Safety and health Act (OSHA) requires that all volunteer firefighters are physically capable of performing duties which may be assigned to them during emergencies. All applicants must:
• Be free from any known heart disease, epilepsy or emphysema (unless a physicians certification is provided)
• Pass a pulmonary function test
• Pass a routine physical examination
I understand that any false answers, statements or implications made by me on this application or other required documents shall be considered sufficient cause for denial of membership or removal from the active roles of this department.I certify that this application was completed by me, and that all entries on it and information supplied by me are true and to the best of my knowledge.
NOTE: A membership fee of $3 must accompany this application. (This will be a yearly due as well)
